Z-Forms are templates created by a team of physicians and certified professional coders, to assist the physician in documenting patient encounters. The forms are designed to adhere to CMS guidelines, eliminate transcription costs, pass an audit, and allow the physician to spend more time with the patient.

ROS Review of systems
PFSH Past family and social history
Past medical history including medications
Physical Exam
Benefits of using Z-Forms:
Dictating will no longer be necessary, as our forms cover all components of the 1995 and 1997 audit tool
Z-Forms will allow you to spend more time with the patient
Transcription costs will be eliminated (how much are you spending?)
Transcription errors will no longer be a risk to your practice
Documentation will meet CMS guidelines
Z-Forms allow use of either the 1995 or 1997 guidelines (can not mix).
We currently have the following packages available:
PM&R (combination forms available)
Internal Medicine Orthopaedics (includes short form and x-ray forms)
Family Practice
Most packages consist of New Patient forms, Initial Follow-up forms, and Subsequent Follow-Up forms, and all packages include a selection of Administrative forms. The PM&R and Internal Medicine packages also include a selection of Inpatient forms.
